Initial Comments:
An off-site, state licensure follow-up survey conducted on July 9, 2021 found that Paragon Home Health Care Corporation had corrected the deficiencies under 28 PA Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a state re-licensure survey completed on May 13, 2021.
